We are happy to bill your insurance company, but please be aware that our providers may vary slightly by policy and contract. We will check insurance benefits prior to the first appointment; however, it is the client’s responsibility to be informed of their coverage. The benefit information provided to us by the insurance carrier is not a guarantee of payment. Clients are ultimately responsible for payment of all costs not covered by insurance. Payment is due at the time of service.
Please note that we have a 24-hour cancellation policy for all appointments. Cancelling less than 24 hours in advance will result in a $75 no-show fee.
Keep in mind, if you have a deductible, you will pay the full “allowed amount” before your insurance will cover any part of the cost.
